Dr. Provenzale is a 1988 graduate of Loyola University of Chicago School of Dentistry. He completed a medical residency in Anesthesiology at Advocate Illinois Masonic Medical Center in 1990. Dr. Provenzale is an Honored Fellow of the AAID and a Diplomate of both the National Dental Board of Anesthesiology and the American Board of Oral Implantology/Implant Dentistry (ABOI). He currently serves as Vice-President of the AAID and on the Executive Board of the American Board of Dental Specialties.
