This lecture will show how to combine the best of both worlds: guided surgery and transcrestal sinus floor elevation using drills for osseodensification. Using a digital workflow, the surgical protocol for crestal sinus grafting can be incorporated in the virtual planning and a surgical guide can be designed that allows for using the densifying burs and surgical keys of a universal guided kit in incremental steps to facilitate compaction autografting in both lateral and apical directions resulting in adequate elevation of the maxillary sinus membrane and subsequent crestal sinus grafting with autogenous bone or in conjunction with allograft or alloplastic putty. The lecture will illustrate how to interpret radiographic landmarks to identify the appropriate surgical protocol. Clinical case presentations will show the step-by-step process to safely and predictably perform guided transcrestal sinus floor elevations.
